HOW PAINFUL IS IN VITRO FERTILIZATION (IVF)?

                                                WHAT IS IVF?
In vitro fertilization (IVF) is one of the most reliable treatments to infertility. IVF treatments consist of cycles. The process involves egg retrieval, fertilization with partner’s (or donor’s) sperm in the laboratory, and placement of the embryo(s) into the uterus.
Couples who have failure becoming pregnant with other fertility treatments usually turn to IVF to conceive. However, most women would refuse IVF option due to fear of pain. This has therefore prompted the question of “How painful is IVF”?
    
IVF CYCLES AND THEIR ASSOCIATED PAINS
The IVF cycles consists of several stages occasioned with varying degree of pain. People experience sensations differently as each patient has a different reaction to the procedures involved, and may or may not experience pain. A successful IVF cycles implies childbirth, so you can’t rule out the absence of pain whatsoever – it is definitely worth it after delivery.
    
So with that in mind, here’s a breakdown of what you should expect, pain-wise, at different stage of the IVF cycles.
  1. Preparation Stage – This is the first part where the woman anticipates pain, as she begins her injections for eggs development. This procedure is usually done with either needles with small bores or the newer injectable pen devices. Most women dreads the first injection for fear of the unknown, but immediately the initial anxiety ends, majority of the women say that there was little or no pain.
  2. Egg Development Stage –The woman also complain of pain as the eggs starts to develop and the ovaries begin to enlarge, resulting in some lower abdomen pain and bloating, especially when large number of eggs develops and the ovaries become over-stimulatory. However, if stimulation is well-controlled, many women don’t even experience any pain, but slight discomfort that they are able to cope with.
    
  1. Egg Retrieval Stage –Many women anticipate great pain during egg retrieval, as they are aware that the eggs are extracted by piercing the ovaries through the vagina using a thin long needle. But in reality, there is no pain during this procedure as it is carried out under anesthesia.
  2. Embryo Transfer Stage –The embryos are usually transferred back into the uterus three to five days after egg retrieval. The transfer of the embryos is almost painless; however you may feel discomfort only during the insertion of a vaginal speculum, which is similar to that felt while taking a pap smear.
After the embryos have been transferred back into the uterus, the patient will begin on progesterone, which are either injections, gels or suppositories. The administration of progesterone injections are more painful because they are oil-based injections and hence the needles used have larger bores. Some patients can bear the pain while others can’t tolerate the severity of the pain. This is why at our IVF centre at Zeeva Fertility we do not routinely recommend the use of these injections or any injections post-transfer, hence resulting to the use of only non-injection forms which are painless, such as oral or vaginal preparations.
     
Pain is always a possibility when undergoing a medical procedure, and going through IVF treatment is not exceptional.
So in summary, with respect to physical pain, while IVF may have the potential to be painful in a few stages, in reality, there is minimal pain or mild discomfort for majority of patients.
However, nine months later, there comes the much greater pain associated with childbirth and the postpartum period, which far outweighs any pain of the IVF cycles.

What is Egg Freezing?

Egg freezing or so commonly called mature cryopreservation of oocytes (women’s eggs) is a technique that was developed to enable a woman’s egg to be extracted, frozen and stored as a method to preserve reproductive potential in women. A woman may decide not to have children at an early age, this may be due to reasons such as being busy at work or she prefers to pursue her education at first but may prefer to have biological children in the near future hence the need to undergo egg freezing process.
For egg retrieval, a patient undergoes the same hormone injection process as in vitro fertilization. The difference between the two is that in egg retrieval, they are frozen for a period of time before they are fertilized and transferred to the uterus.
Egg freezing consists about a week and a half of the hormone injections. The reason for this is to stimulate the ovaries to produce multiple eggs in one menstrual cycle instead of the single egg they would typically produce. During this period, there are about a week of short visits to the doctor including blood tests and Transvaginal ultrasound exams. This will help assess the individual’s response to medication and possibly make adjustments depending on how the eggs are progressing.
When the eggs are mature and ready to be “harvested” is when the surgery will take place. The surgeon will retrieve the eggs by insertion of a needle through the vaginal wall. This will sound grueling but the patient will be on anesthetics so no pain. As it is done by a needle there are no cuts whatsoever. The eggs will then be frozen using vitrification.
Shortly after your unfertilized eggs are harvested, they are cooled to subzero temperatures to stop all biological activity and preserve them for future use. The makeup of an unfertilized egg makes it more difficult to freeze and lead to a successful pregnancy than does the makeup of a fertilized embryo.


Age if a factor to consider when considering egg freezing. Women who undergo the procedure before they are 30 show the highest live birth rates. Therefore those considering egg freezing should do it when below 30 years to increase their chances of having live births.
Egg freezing has its own advantages making it one of the growing and trending technology in the medical world. One may decide to do egg freezing if she is about to undergo cancer treatment or another illness that may the fertility potential in future. Medications such as chemotherapy may harm a woman’s fertility. With such diseases egg freezing may be an option to have children at a late date when they have been cured.
As women age, the quality of their eggs diminishes. Just as the likelihood of conception in women decreases each month, so does the likelihood of having eggs with abnormalities. With egg freezing, women are able to protect the current health of their eggs and increases their chances of conception though IVF in the future
